Controller issue?

Tried playing a few nights ago, first time really, and I kept having to connect and reconnect the usb cord to be able to press start at the title. From there the first mission mostly goes ok til I get to the helicopter boss where my controller disconnects. I try to plug it back in, get blown up, plug it back in and it starts vibrating violently. I have to close the game via task manager and then I set my controller on the desk.

It vibrates for roughly 3 minutes until I finally unplug it. For reference I'm using an xbone controller I've had for several years. Is this a common issue or should I replace my controller?

Try getting the Xbox Accessories app from the windows store and doing a firmware update, I was having constant disconnects until I did that
