Beat all three endings and pretty positive i have all missions unlocked, however no stargazer achievment?

There are 59 missions in total. Have you done all of the alternate missions as well. I am not sure if they are automatically unlocked in NG+ and NG++

some existing missions have an alternate version once you're in ng+ or ng++ so you gotta run em again

There is no new game 3, and if there is, there aren't more missions that add to the achievement. I completed Main Game, NG+, and NG++ and unlocked the achievement.

I count 59 missions (Have cheevo) can count by clicking down arrow.
I had to double check missions (In NG+ & NG++) with decisions in them via replay missions to make sure I choose the missions I had not done.

The dam mission & the stealth mission both have options within them that I think add extra missions.

If you make suboptimal choices with decision missions, you may have missed one. For example if you choose to kill the cataphract in 2 of your 3 playthroughs instead of killing the HC at least once, you might have missed that mission
