(BUG) "Nephro, Captain of The Guard" not spawning

I cant really give much insight on how i think this occurred, but when I got my first shell after triggering the vending machine and taking the tin can I could just walk right out and proceed with talking to the duchess. Please fix this if you could.

I had a similar bug. When I took the tin can the fist time the boss got triggered, I died, respawned next to the vending machine, got the tin can again but the bossfight didn't trigger, I just walked out, came back later and the fight still didn't trigger. I just progressed through the game and came back later, eventually it got triggered again.

Happened to me too. I killed myself and respawned to fix the issue.

Same happened to me. I had died multiple times and responded at that location. Then some how he just stopped responding. I had to start a new save because of these bug.
