Drunken claw Attack Power increase plus charged heavy with fork hammer does stupid damage, and increases parry/riposte damage
Your build is fine, for now, a bit glass cannon for my tastes.
When I first started playing souls-like games I always went for the higher health at the start but as I've gotten more experienced I go for the higher damage to rely better on my reflexes and move set knowledge & put some health in later if I feel I'm getting too overwhelmed.

Stowaways: SInker+ / Sinker+ / Lamprey+ (70% Balance Increase + 7% Life Drain)
Favorite Adaptation: Tactical Tentacle (At LVL3 lasts longer & gives health on hit)
By the end game I was able to pretty much just tank hits while trading with the bosses and massively building balance and regaining health.

Most normal mobs die in one hit with this, with exceptions for the larger ones. It takes some clever use of parry to really get the best use out of it though, especially against bosses, but it hits like a truck. Didn't really play around to see if charged hits was more worth it for me over raw hammer damage.
