At a guess, it might be a CPU limitation. Streaming takes some CPU usage as well, so it might be having trouble running the game and streaming at the same time, with one getting in the way of the other and causing a small hitch.
I have no idea if the results would be any good, but you could try setting the affinity for the game to not use one or two threads, then set the affinity for Discord to only allow those threads.
