Think of it like Dungeons and Dragons. When a few guys jump into an empty server, a Game Master must be nominated first who'll oversee whatever scenario he wishes to create. He'll create the factions, the spawn points, lay down ai enemies, vehicles, etc etc, and then get everyone to spawn in and enjoy the game. The GM will be watching like a god from above ( old Arma was Zeus ) and try and create moments on the fly as the players fight.
He can also upload a GM scenario he has created on his own PC, all ready to go.
I often jump on first, nominate myself as GM, and then ask the other players to give me 10 minutes to set up while I set things up. It's great fun - like being a movie director.
I now do it on a private server to make sure strangers dont jump in.
Also, the GM can 'puppet' any AI enemy and take over to join in the fun and create chaos and interesting dynamics
