And different types of fish have different types of quicktime events.
There's also a "bug" (doubt it's a true bug honestly) that if you shoot a fish near a wall with the harpoon it'll just bounce off without damage.
And yes. Fish have a HP system, and depending on how much damage you do to the fish depends when you can instantly grab it, activate the "mash", or just damage it. Once it hits a thresh hold for HP you can then get the minigame.

And I wouldn't say its a bug, probably would place the fish in a wall if it breaks free.
For me it does deal damage, just bounces off the wall
However is it me or is the electric QTE broken? like I got not time at all to start mashing. I barely get a chance to make out what buttons it wants me to press!
Is the electric button mash QTE broken or not. I cant even see what buttons before its failed.
It's not broken, cool your jets.
It's a fast-reaction mini-game; that's just how it is. Your best bet is to memorize -- either through your own explorations, or by googling -- what the button-mashing QTE is for each given harpoon tip. Then, when you come across one, you'll know what to do.
Personally, I cannot stand the button-mashing QTEs, so I only use the standard harpoon tip and its upgrades. You can play the entire game without ever picking up one of the 'fancy' tips.
Some of the reaction buttons are not about mashing but a sequence. Like say if you harpoon a fish and get a prompt like A W D then you press A W D in that order and you will get the fish.
