You probably may need to craft it from either your gang's bench or somewhere else. Are you looking for the formalin bottle or the finished item that uses the formalin bottle as a crafting ingredient?

I have both the bottles you craft with, and the empty one you can throw as a weapon. My bench is maxxed, can't max out the bench at the gang yet. I think I had the bottle once, but threw it in a fight like an idiot. Lol.

You need an empty bottle and toxic fluid, to craft it at the Gang HQ (it is a tier 2 item, if I recall correctly). The latter can be found in the outer area of the prison, once you have won your first gang challenge and received the restricted access key card. The Science Lab, in particular, has it in abundance. You can access it either through the Medical Bay or the Bathroom's restricted door. You may sometimes find it, as well, in random encounters close to the Lab.

You will need a screwdriver to open the containers that have it. Alternatively, the Lab has a way to convert gunk to toxic fluid.
