The Soul mode upgrades mirror the boss rewards exactly, so it's either +3 points to a random attribute, a random amount of gold, or a random Power for that run.
The difficulty scales, more or less, with the level of the dungeon you're in. It varies a bit per mode, but it does tend to get harder the further you do in the spire. If you're in the same floor as a previous run in the same mode, then overall the difficulty should be the same.
All gold picked up, including coins, is stored in the money bags. To combine items you simply drag two of them to the Combine Slot, and a new item will pop up there. The first item placed there matters for the result, and all items can be combined somehow.

And how do you move around items on your belt slot? Neither Interact 1 nor Interact 2 do anything besides use or unequip. Using XBox controller.
To move items around, you have to enter the inventory and just drag and drop them with the cursor, using the analog sticks and the A button.
A quick key for combining is the Middle Mouse Button.
