Cultist Portal not working

It's the only ending I have left that I'm aware of, but I am unable to get it in my run and I have no idea why. I put enough items into the portal for it to get to 500, I put the phlemp card, an item from another run, and the bobblehead but the "item once repaired" does not work. It is never crossed. I put items into the auto repair storage overnight then throw them in, and it does not get crossed off.

Which means I can never complete the ending, and I have no idea what to do next. Am I missing something?
