Blocking on controller (LB button) not working

I'm wondering if any one has come across this issue with you controller registering the LB and LT button as the same thing? It LB should enable me to block, however when pressed it activates the skill hotbar, which is what the LT button also does.

I looked through the steam controller settings and even checked a gamepad tester, and both show that the buttons are configured and registering correctly. To be sure, I even tried with a second controller and the outcome is the same on that one.

Frequently when I start the game now it reconfigures my controller buttons and I have to go to the edit layout option in the steam controller settings and fix the right and left bumpers. It wasn't like this on game launch. I have other controller bugs that were only introduced after the first patch.
