In DA2 they got to design and flesh out the qunari more. They also redesigned elves to look more distinct from humans, but because people disliked the drastic changes they rolled that change back a bit.
Then in DA:I when qunari became an option for player characters, I think they wanted to give players the chance to create more human looking qunari in order for people to more easily connect with their characters.
To be honest DA2 Qunari had the best design to be honest
I would like for the creators to come up with a decent explanation why the qunari have horns. maybe they got cursed? it would explain why those born without horns, might be seen as blessed?
From the DA wiki:
"Hornlessness is a rare genetic variation in Qunari, akin to red hair in humans. Those born without horns are considered special and are often given prestigious roles in Qunari society such as a Ben-Hassrath or an envoy to the other races. Sten, who became a companion of the Warden as a soldier of the Beresaad, falls into this category. Culturally, Qunari associate not having horns with being imposing or scary, and because of this Tal-Vashoth often decide to remove their own horns. Likewise, Saarebas, the Qunari mages, have their horns removed to warn of their danger."
That explains the "blessed" part at least. Though there isn't any explanation for the horns existing, other than they just do, and some have several pairs.

So, I join the consensus that the qunari looked best in DA2, and looked worse in DA:I and DA:V, and of course worst of all in DA:O.
One thing about DA2 qunari, however, is that they all basically look identical, and horn shapes and paint are used to distinguish individuals (a-la anime character eyes and hair).
So once BioWare decided to make qunari a playable race, I wonder whether it proved too difficult to allow character creator customization using their more alien DA2 features. So rather than taking the time to create a completely separate character creator for qunari, they watered down the qunari look to be more human. Sad, but perhaps understandable.

It also gives me hope that even in DA:I you could create both more rugged looking qunari and something more similar to what was now show, not to mention some of the female qunaris created look exactly like big humans with horns (smooth skin included).
Oh god. I need this positive mind set.
Or...work with me here....the player who created that specific qunari wanted to make a prettier one. We don't know that there won't be ugly or more alien options. Yet.
Personally I feel the Rooks they've been showing us so far has been to let us know that things everyone complained about on past games has been addressed. We saw a black/tan rooks to show us that they've gotten way better on darker skin tones than all their other games.
There were also tons of complaints back when Inquisition came out about how ugly qunari options were and how hard it was to get a cool looking one. Especially the hair. And say what you will about the face in that demo, his hair is spectacular. Seeing moving hair was what got me excited in that clip.
Can't wait to deep dive into the creator personally.
I mean, I am SURE that Ch-ad, secret agent of the Bro-heemsrath, is an outlier. I just meant in general that the Qunari of DA:I and what we have seen in DA:V so far are more human in appearance.
But yeah, DA2 had the best Qunari. They looked fierce. These qunari look like Tieflings that escaped D&D.
I think the reason some qunari look more human than others is an artistic license issue. Like when we want something to appear more hostile or threatening we make them less human, but to be seen as more friendly we make them less. It's actually a common tactic in science fiction.
I would bet that more traditional DA2 style Qunari exist in Veilguard, but theyre more or less part of the Antaam.
The third thing it might be because DA2 is a framed narrative and Varric kinda views all the qunari as more monstrous than they actually are.
