Well two things. First only Morrigan knew/taught shapeshifting. Also, you didn't pay attention to Morrigan when she told you about shapeshifting and how it worked.
Would you like to quantify how easy shape shifting is?
1) You need to be a mage or hire a mage.
2) You need to KNOW the spell.
3) IT DOESN'T LAST. At least so far as I recall. Or is Morrigan still running around as a spider in your game? The only other shapeshifters I can recall are the werewolves and they were cursed.
So I don't actually see that this breaks with lore. But even if it did, doctors are probably cheaper than mages who are all locked up in their towers (so far as most people know), and are probably not cheap to hire out if you want to hire a demon to come work on you.
ETA: Oh, wait, I forgot this is Tevintir. The mages aren't locked up. Just if you bring yourself to their attention, they might drain your blood and your life force if they're having a bad day. They are not people you want to pay to do anything.

Or maybe you don't involve a mage at all? So you have scars.
