I've done all requirements to get the secret ending but I am missing a codex entry :/

I have the same problem. Pretty sure I "talked" with the creepy voice 3 times; went back and checked: no magic circles left.

Unless the "magic circles" disappear after some time, looks like a bug.

Interesting that 15% of players got this, although that looks to be about 1/2 the people who finished the game

btw: AFAIK, order doesn't matter, so Arlathan is not necessarily the 3 codex entry

Check Nexus mods,I uploaded a save that will have this unlocked. Not on PC so I can't give you a direct link but it should be easy to find
