If it's a part of the model someone would need to make a 3rd party hack/model replacement. Odds are they didn't bother to make anatomical elements beneath the heart since there's no point in sculpting them with Japanese censorship laws.

I saw a Patreon account that say the game has an uncensored patch.

Basically the adult patch isn't what you were looking for as it only adds the Explicit Scenes button in the menu where you can change their costumes and gain direct access to those scenes without running through the Chapters. The only useful I found is a Patreon offering a proper patch. -> https://www.patreon.com/posts/hf-patch-for-v1-83668849 (but it's paid content though).

Hey there. Thanks a lot for posting the link to the patreon. I subbed the guy to get the direct links. I might as well share it to everyone.

Installation instructions:
Download all parts to the same directory. You can mix parts from different sources, but the file names need to stay the same. You can use any directory other than the game directory.
Start the included .exe file. A UAC prompt should appear. When you click Yes the patch installer will start.
Follow instructions in the installation wizard (for most users all you need to do is keep pressing Next).
If the installation location was not automatically detected or is wrong, you will have to show the patch where your game is installed. If you bought the game on Steam, go to your steam library > properties of the game > view local files to find where the game is installed.
Warning to Steam users: Even if you want to only play in the NonVR mode, you still have to install the patch directly inside "KoiKoiMonogatariVR" and not in the "NonVR" folder.
If the patch download is corrupted, the patch will show an error message or ask for a missing disk. If that happens: close the patch, download it again (try using a different download link and/or web browser) and retry installing the patch.
After the patch installation finishes you can remove the downloaded patch files. That being said, I suggest keeping them in case your game breaks for any reason and you need to install the patch again to fix it.
