When I played it for the first time, I was surprised how much more health enemies had. I just barely lost with a mutual K.O at the boss of second act. The worst part was that I had 5 Flame left when I entered that fight. Had I been less greedy earlier, I could have gone past that boss.
When in doubt go Ethereal. Ethereal builds might not be the easiest to build, but they can deal a lot of damage. They benefit heavily from things Ethereal Amplification and Omnicrit combined with Critical hit chance & damage.
Consider picking heroes with powerful reborn abilities that you can use straight away. You want to visit campfire in Act 1 to even up the odds a little, unlike in regular runs where rushing campfire is often the wrong choice to make.
