I had the same problem. Let me tell you what fixed it for me.
From the main menu, go to Options / Sound / Environment Settings and and try to select Stereo instead of Multi-Channel.
There's probably a proper way to fix that but I'm too lazy to dig more.
FYI, I have razer kraken ultimate headphones with 7.1 sound.
Maybe that's related to this issue...

Ran into similar issue in some cutscenes. I can vaguely hear as if they're in another room instead of the room were seeing them in cutscenes.

I had the same problem. I switched my Options -> Sound -> Environment Settings to Stereo, and after an Alt+F4 during the main cutscene, relaunched the game. It still wasn't working, so I did another Alt+F4 and verified the integrity of the game cache. After that, I confirmed the Environment Settings was still set to Stereo (it was) and the issue was fixed when I started the game again. Hope this helps.

So, I am also having the same problem as well. You can turn on Windows Sonic to help out the situation. You hit right click on the Volume Icon and it should say Spatial Sound. After that should say Windows Sonic.

I'm having the exact same issue as well, it plays 100% fine through my regular surround sound speakers setup, but the second I want to use my headphones suddenly I can't hear the cutscene audio. All the audio outside cutscenes is perfectly fine, and noise. I'm thinking it's something that they'll have to address in a patch

So i figured it out, I use an usb to split my headset and my headset mic so i unplugged it and only plugged it in into the speaker port and the voices came back. cant talk to friends while playing it tho D;

Yeah it seems to bypass audio enhancement, which stops any drivers from mixing the surround sound properly.

I've sent a ticket to bethesda support
