It suppose to be surreal, as it is happen in her dreamscape...
it's just her head, either the back of it or the front without a face, in phase 1, Phase 2 is the single eye of the Tianhao virus mutation... and the Phase 3.... I got nothing on the Butterfly with eyes on it's wings.
It's meant to be abstract, showing how her guilt, has caused her mind to fracture into mental instability... partly as the dreamscape project isn't perfect and causes the Solarians to lose sanity the more they are in it.

I think the third phase is meant to represent the thing that Lady Ethereal thinks the Council is guilty of...we see all these cocoons holding her previous team in them while the big head behind the cocoons has the single eye of the Tianhuo.

So Lady Ethereal's subconcious is trying to process the guilt of being part of a Council that, instead of saving their species, trapped all of its members in a never ending dreamscape while they slowly get consumed by the virus, too lost in "perfect dreams" to ever want to wake up.

I do think they could've illustrated stab wound a bit differently, a bit more diagonally and all 🤔
