Has this game
Have difficulty options to choose? If yes, can it be changed during play, or only chosen at the beginning?

The game has two difficulties: Standard and Story.

Standard mode is the intended experience for the game. Story mode lets you adjust the damage you deal and receive to whatever you want. You can switch between modes whenever, but switching to Story will cost you an achievement.

You can only switch from Standard mode to Story Mode, not vice versa.

Yes. Note that you can tweak the combat difficulty to be the same as the Standard Mode in Story Mode, simply adjust attack/damage modifiers both to 100% will do.

Is it all achievements or just the one specific to completing standard mode without dropping to story? I beat standard and got the achievement but I'm missing quite a few achievements. I'm thinking of going back on story mode for the rest.

Logical would be to be just this one achievement... But maybe better way to wait for someone well informed to confirm.

Just this achievement in particular depend on Standard Mode, use the Story Mode to obtain the others all you want.
