Unexplained Death
I was with a teammate, and while attempting to pilot the ship out with an intruder aboard I suddenly collapsed as if I ran out of oxygen. My team mate claims I was hit by a bullet in the head, but the intruder was our of the room and had no vision of me. I had no explanation of death, only told the basic space is dangerous line. Has anything similar to this happened to anyone else? Bug or cheater?

Ricochets happen from time to time, grenades sometimes bug out and you don't hear/see the explosion and they won't show up in the death screen, and there is a bug that can cause random death that I have had happen to me twice. Could be one of those. Could have been an enemy player shooting you in the head, too. There are a few spots where you can fire a shot into the cockpit and hit the pilot in the head but your team mate wouldn't necessarily have seen the person if they weren't at the proper angle.

If I kill myself with a ricochet I get my own names displayed, but if someone kills me with a ricochet then not?

The end game stats screen is also buggy as heck. I've explained it in other posts but if you fall and take damage you get a "hit" landed which increases your hit %. Using the flamethrower means all shots you take with it don't count toward shots fired so your hit percentage can be 2000% or 3000%. Sometimes bullets penetrate and strike multiple things. Shotguns count 1 shot as a shot fired but each pellet that hits counts as a shot hit, so you can easily get 400-500% accuracy when using a shotgun. Sometimes the damage numbers don't make sense either.

Depends on whether or not the end game stats screen bugs out. Certain explosives can be bugged and not display who or what killed you. AP mines used to display the weapon the person who placed it is currently holding, so you could die to an AP mine but it might show a Thompson or STG-44 or nothing if their hands were empty - or if they left the match before you stepped on the mine.
