It also depends how long you were on your journey throughout the day because sometimes when you respawn it will be later in the day.
No matter what every map will have a different generated layout and different loot in every loot container.
OP is talking about the "permadeath" option that you can activate.
And yes, it's not working, you will have to manually delete your save like I've been doin' for a while.
I hope they can fix it or else the "Ironman" achievement is gonna be one hell of a joke.
Dunno if you didn't know, but both permadeath and drop ALL gear on death are available in the difficulty settings.
Playing with permadeath would be for absolute masochists or perfectionists imo - playing with drop all gear on feels more appropriate for an extraction shooter, but definitely not best for when learning the game (you can easily spend ages doing forest runs just grinding to stay geared and losing it all again and again).
