My go-to is simply Survivor preset with loot amount, loot amount per item, and NPC ammo cut in half. I still find too much loot in the mid- to end-game phase, though.
This system allowed me to lose gear sometimes and also increased difficulty slightly because I had to dedicate a stash tab to insurance returns.
Also, feel free to put weapons/gear in whatever tier you feel is right. The current tiers are just my personal preference and items I didn't regularly use are likely missing. For d20 rolls you can type "roll 1d20" into google.
Additionally, I haven't played this latest patch. Jumping into it today, so there might be some items/changes I do not account for.
Hard Mode:
Safe Item increase levels. If you die select the number of squares that correspond to your level and move items you want to keep into them. Drop all other items. This simulates the secure container from Tarkov. Allow guns for these slots or not based on your personal preference.
Lvl1 - 4 slot, 2x2 size.
Lvl2 - No increase
Lvl3 - 6 slots, 2x3 size.
Lvl4 - 9 Slots, 3x3 size.
Lvl5 - 12 Slots, 3x4 size.
Gear Limits
Insurance:
If you die you roll insurance based on the gear on a d20. If you pass the check, you can use the gear the next in game day because no one else looted it and it was returned to you. Store it in your stash until then. Harder check the higher the tier of the gear. If you fail the check, drop the item on the ground. Unfortunately, there's no way for this to impact your funds since you don't actually buy insurance. If you want to be really hardcore about it, you could buy stuff in the bunker and just drop it off in a corner to lower your money to simulate paying for insurance.
Tier 1 - roll a 2 or higher
Tier 2 - 4 or higher
Tier 3 - 8 or higher
Tier 4 - 12 or higher
Tier 5 - 16 or higher
Tier 6 - 20 or higher (only possible if an animal killed you, so you almost always lose
tier 6)
If you were killed by an animal add 4 to the roll. This is to determine of someone randomly came across your body. It basically moves any item down 1 tier (for higher tier items) in terms of roll difficulty.
No one will steal a basic head lamp cuz it looks dumb as hell. :P

Set the Survivor difficulty with the following changes:
Trading:
Trader's Money: 2.5 instead of 1
Items Amount: 0.5 instead of 2
Price Weapons: 2 instead of 1
Price Armors: 0.5 instead of 1
Price Backpacks: 0.5 (Although prices seemed to be bugged and don't work)
Loot:
Minimum weapon durability: 0 instead of 25
Maximum weapon durability: 60 instead of 45
Progression:
Character Health: 80
Minimum weight: 11.25
Reputation multiplier: 4 [optional]
Reputation from quests: 4 [also optional]
Hardcore:
Lose Ammo, Lose Medication and Lose Consumable: On
Lose Equipment: Either On, or Off - but if you play with Off, you MUST discard your weapons upon death (meaning you can only keep backpacks and armor - once they fix price modifiers, turn this On).
Losing keys is optional, I recommend Off.
Lose other item i recommend to keep On to prevent tasks from bugging out.
Must Choose 'Equipment 5' at the beginning.
House rules:
At the start of the game, drop ALL items you hold - yeah you're starting naked.
You are not allowed to get free items from Barman.
You are not allowed to get guns from tasks. Immediately sell them when you do.
You are not allowed to loot Igor.
You are not allowed to sell guns at the vendors.
You are not allowed to bring guns under 30% durability with you on missions.
If you buy any gun at the store... remove ALL attachments (you can sell or drop them).
Whenever you take off a mod or mods from a gun you have to either:
A: Sell/Drop all of the mods you removed, or
B: Choose ONE mod to keep (sell/drop the rest) but you also must scrap the gun.
When repairing weapons with either a big repair kit or a work bench.. you must discard ALL mods from it after the repair (and drop/sell them).
You are allowed to "Buy" a Bulletproof Vest from Barman or Doctor, by buying and dropping either the laboratory key, or Repair Kit For Radio Tower on the ground - they both cost 5000 (after you buy and drop them, simply spawn a bulletproof vest via in-game debug menu)
You are allowed to change the weather if the area you entered immediately started off with radioactive rain [optional]
Win condition:
Getting the Kibba quest and eliminating Kibba once.
Lose conditions:
You die for the 4 times.
1 in-game month passes (see date on PDA, if it goes to February the game's over).
You go over 60 hunts.
Good luck. It is beatable.
