Saving Everyone

Even after replaying the final segment at the train station several times, I've been unable to figure out how to save Anders, Esther, <i>and</i> Liva.

According to some of the stuff I've read all I supposedly needed was to have medical supplies for Anders.. which I do, but it wouldn't let me save him after the explosion. But if you choose Anders during the final set of choices, either Esther or Liva die.

Anders was in good condition when I spoke to him cause Reinhardt had made sure he wasn't tortured too much. Kind of a huge pain since I managed to save everyone else, you would expect there'd be SOME way to make sure nobody (who doesn't die by default) is killed.
