Not only i have no more stutterring at all it also fixes an input problem where if vsync is active additional inputs when holding notes are registering on "key up" instead of "key pressed" which was making me lose the first input after hold every time
I have one named High Frame Rate by MandM but the problem is still there :(
Also, I've recently noticed my Nvidia's extra software seemed to be causing slight stutters. Once I disabled it, they stopped. Go to Windows Services, then find and Stop "NVIDIA LocalSystem Container." You can then re-enable after the game if you want. My stutters from this weren't as severe as yours sound, but it makes Nvidia a common link here.
The most consistent way i've found for best performance is to downscale the display manually (in the OS display parameters).
On my 4k monitor I've downscaled to 2560*1440 (no frame) and the better performance helps a lot with the extreme charts, it feel as if the game is a lot more responsive .
possibly a disc issue tho.
Never had this issue until I recently upgraded from a GTX 1060 to a RX 7900 GRE. Pissed off beyond measure!
Edit: Only fix is playing in windowed :////
