Need help with a few things
1. How do I get the maid costume for pig-man mansion
2. How do I reach Jigoku? Fortune teller says that's the next area
3. Where do I find the nurse for the clinic in Spirit City?

Pig-Man and Nurse Costumes are found in the Fungal Forest which is down and right from Spirit Castle.
Jigoku you get to by passing the vines to the left of the Shady Alley in Spirit City.
You can buy the nurse costume in a couple areas.
