Otherwise I do agree, its an awesome concept that, if applied in ways that weren't mandatory to progression would be super fun and unique, but really annoying when you get to that point where you have done everything and are cleaning up the endings and realize your options are "wait until November 27 to get this ending" or "change my computer clock so I can continue enjoying the game while I am still really into it".
If I had a suggestion on this, I would say just make that mechanic only affect a ton of of flavor stuff in the world. The new enemies, some locations have different colors/extra sounds, NPC's doing unique things around town, stuff like that, cause that mechanic in itself is really cool and interesting, but locking important stuff behind it can easily lead to situations where people just find it frustrating.

There's a weapon that talks about only awakening its full power or something during a full moon, and the manual itself explains the magic system stating that some things get better with a full moon. That's a pretty strong hint that there are multiple things tied to IRL moon phases.
magic grants more lunacy, which causes you to take more damage, but you also get more xp and deal more damage with spells. You can also increase lunacy by using liquid moonlight potions, which are a pretty common drop from phantoms and have easy crafting recipe.

These are most noticeable differences that aren't cosmetic
Either way there's a mod now that does exactly what I suggested, and changing system clock isn't exactly hard to do.
Still, would've really liked if there was a hint about any of it. Considering what ending it's important for, it would've been fitting for a book in either the Temple of Silence (which is fittingly at the beginning of the game) or the Labyrinth of Ash to hint at the fact that there's more than just stat things that happen during a full moon. Maybe even books in both locations, with a Temple one hinting at the enemies and a Labyrinth one hinting at the item.
