HELP!! 98% Complete Extras 57 of 60

I'm completely lost & can't find what I'm missing. I've been following Kilroy's PDF Guide for the most part. I've been going over the Extras (starting @ page#248) & still can't find what I'm missing. I have crafted the ??? & given it to Madelyn. I got the crafting ALL achievement. Thanks for the help!!

Download my guide and go to page 238 and figure out what you are missing. Most of the stuff you need to find will have the location.
