Crumpled Application Form - [Thanks to Idana37] When listening to gossip by the doors in black street, one conversation is a man and a woman saying they are going to loot in the bazaar. After that go to a door between citizen square and Vulcanes blacksmith, where you can listen to hear the man and woman talking about the cult. You can choose between speaking up and convince them to go back to black streets, or you can stay quiet. If you stay quiet they will mention going to a relatives house near laurel crafts. At the lion plaza where you had to dodge eulalia's feathers you can go through a ruined house towards the granary outpost. On the way you will see a warewolf and a cultist mage. If you kill them there will be a small object under the rubble that gives the document and a quest item.

Missing document in Phalan Academy is called [Beta8's Testament].
Localization: Where Beta8 is sitting, near the second elevator.
Condition: Accept Beta8 quest and defeat Twin Colossus before retrieving the item / completing the quest. Important this might lock you from Minerva's Glory ending.

Unnamed Record in Stella Mine can be found near miner with green hammer in Hollow. This miner also drops key to foreman cabin.

I found a source for two of the final documents. All credit goes to this French Youtuber by the name "L'abri de Kane" (https://m.youtube.com/c/LabrideKane/videos) for finding the final Deeps and Caelum docs.

The Glittering Deeps one is Tanrius' Research Records #4, found on a ledge just to the right of the pink crystal doorway leading to the Titan.

The one in Caelum Summit I haven't personally obtained, but it should be titled "Letter from a Senator" or something, and it's just to the left of the Senator's Oath of Office (facing the stele), along the ledge, right next to the rock of the mountain.

Those are actually #1, #3, and #5. First two (#1 and #3) were available the first time I was there. The third one (#5) appeared later in the game (can't say for the exact trigger) behind a broken wall.

I was just past that spot on a new playthrough and #5 was there on the first visit. I had Quintus quest and I killed the warewolf.

I found the last missing document in bazaar. When listening to gossip by the doors in black street, one conversation is a man and a woman saying they are going to loot in the bazaar. After that go to a door between citizen square and Vulcanes blacksmith, where you can listen to hear the man and woman talking about the cult. You can choose between speaking up and convince them to go back to black streets, or you can stay quiet. If you stay quiet they will mention going to a relatives house near laurel crafts. At the lion plaza where you had to dodge eulalia's feathers you can go through a ruined house towards the granary outpost. On the way you will see a warewolf and a cultist mage. If you kill them there will be a small object under the rubble that gives the document and a quest item.

Wait, if we actually "save" Odysseus, we dont have access to his note, so its a rip?

I'm sensing a pattern of sorts here... Anyone that is able to, can try go through Stella Mines and don't talk to the wounded guy or talk to him and don't finish his quest before defeating the boss there? The document location is awfully well-fit for this theory and so far all missing documents we found lately are related to not-optimally side-quests results.

But if you don't save him you miss a quest item (the one Bion gives you). So it's not 100% either way
