The dev has spoken about some of the ideas he has thought of if he would implement companions. This is what he wrote about this topic:
"In general, I thought about this before. And if I implemented it, it would be something like this. Very rarely, you could come across events where, say, one of the survivors fell into a trap or is loitering in the house and at that moment he is either hostile to you or runs away or tries to find contact with you. In the latter case, you would have to try to convince him to come with you as a companion. But he would not strive to depend on you and carry out your commands, like he could turn around and leave you at any moment or kill you in your sleep, simply because he didn’t like something or his parasite took over his mind. For this reason, you couldn't raise an army or something like that from companions or a huge following.
Companions would never be completely your companions on whom you can rely. They will either rob you, kill you, or frame you and run away with your loot.
But even if you make friends with this companion, it won’t be for long. Sooner or later, most likely he will be killed by some parasite. And you'll have to start the game over again if you want to see that character again.
Or perhaps it was just your hallucination and there was no companion, all this time you were alone
So far I have these thoughts, and if I sat down to implement companions (living people), it would be something like this. Everything can change."
