Did you get all the bloodfiends and mandrake roots? It's the only two I'm missing. My entire map is full at 99% but I cannot seem to find the last root and bloodfiend

All bloodfiends, yes.
Still looking for the last mandrake root.

Updated the OP with the location of the Queen of Smiles for 'all Smiles'

Ended up finding it. Needed to buy from a NPC...Same with bloodfiend. missed a secret wall

I hope they remove some of the more ridiculous achievements.
Namely, the ones that still have a 0% accomplishment rating even after the dedicated fans spent days making compendiums.

Much obliged for the queen of smiles. Didn't even know you could walk into the water xD. Avoided it because character was like "Not goin in there!"

Same honestly!
It deterred me the first time, but a bit later I was like "nah, let's actually try this!" and was actually a bit surprised you could just push through. :D

It does feel a bit strange to put content behind "I'm going to die if I keep going this way" (almost verbatim) in a souls-like.
