Just died to baldi after collecting like 5 notebooks and it played a weird noise and popped up with this screen
I can barely find anything about it online and It wouldn't really make sense for the game to show this screen if it was a genuine bug from a coding perspective (correct me if im wrong)

if you check in screenshots, you'll see quite a handful of people sharing this secret death screen (example, I personally saw like at least 5 shared including this one)

i believe when attempting to get the secret ending and you lose after getting 4+ notebooks, this screen appears.

Update: This is a secret screen that just has a chance of appearing. There is literally no way to trigger this except for dying.
It literally does nothing else and isnt an actual error.

I got that screen once in demo mode, it seems quite rare, plus there probably is a secret more rare screen similar to the screen but with a bit more lore.
