I thought I saw in another thread that this ending was working correctly, meaning if you reload an old save after you get the code, and open the fissure without the book, you fall into the fissure before Atrus shows up (to show you how sad and disappointed he is). There must be some cases where it glitches.

Logically you don't. You get the trap book with the journal that has the code.

you can open the fissure as the code is in Catherine's journal. I did this first as I wasn't sure if I would get stuck in the trap book when Ghen offers it. Then when I go back and do the steam pipe puzzle (not knowing it opens a fissure and ends the game) Atrus comes through asking where's Catherine and suddenly grabs the trap book, that Ghen still has , from my satchel. Then Ghen comes in the scene from the left with his buddy and shoots Atrus,then me and ends the game

Oh, I see. So you took the extra step of visiting Gehn once after getting the Trap book from Tay, and letting him keep the book before opening the fissure. If I recall correctly, the original game handled this by not having Atrus grab the book before Gehn shows up on the scene, so it worked in both cases. Apparently, they forgot about this possibility when they crafted the endings for the Remake. Nice catch. I wonder if Cyan will fix it?
