there are no hidden or secret endings, each one is represented by a tarrot card. once you have all the cards, you've seen all the endings.

the "second caller" is from the sequence in your house at the beginning of the game. remember how you went to the bathroom to practice making faces, and when you came out you got a pair of phone calls on a pair of phones? /that/ second caller. side note, you already have done this if you got both boiler room endings.
go through it and experience all the possibilities. there's a lot of philosophical things to talk about once you've completed enough.
there's only one thing you'd need a "new save" to do, and that's only for completion's sake, if you want to get all achievements.
without spoilers, there are several main "story routes." basically, the location you go to at the bus stop decides which "route" you're on, and the faces you present during that route can change the outcome of events.
