your guess is as good as anyone's for the janitor. i've seen a lot of discussion envisioning him as some kind of servant for the empress. nothing in game suggests any solution, though.
the waitress, though...... she isnt human. the cafe isnt in the real world. the cafe is outside of time and space, like yu's office (and a red room in another universe,) and the people who work there are something like "minor dwellers."
martha's vision
yea, even lila does not know that one. garage heathen may have had an intention with that one, but no entity in the game is aware of it.
the stranger
yea, there's been a lot of back and forth on that one. people have suggested that the stranger is what's left of will's mother, or a reflection of will's perception of her. other people have suggested that the stranger is some aspect of will's own unconscious mind. there's not a clear answer in the game. maybe the stranger is the mystery of who the stranger is?
the sunshine cafe
it's more of an easter egg than anything else. yet another reference to twin peaks. there is the possibility that the location gets expanded on in future games. after all, the forest of 1000 sycamores started as a not very expanded upon concept in "the amazing t-gotchi."
