- something is parked to close to the joint of switch and adjacent track of the other branch (something passing over the joint is the trigger to throw switches automatically)
- someone is trolling you (maybe unknowingly). By using a mod (I think it is RROX? you should've told) it maybe possible, that someone's aligning switches for his planned moves without respect to other moves (you also didn't mention if you do single- or multiplayer)
- you're running excessive speed and your train is simply derailing (doesn't throw the switch except some part of your train passes over the track joint as mentioned above)
I never have had any problems with switches throwing unintentionally except for the above mentioned reasons. And unreasonable "guests" on the map who purposefully have thrown switches while a train passes over to derail it (guess they thought of it as fun, using password ever since)
I've heard of people with RROx having this happen from time to time, but it's rare. I agree that removing the mod should clear it up.
It is an old bug that often happened with the old track system and I often observed it toggle from a distance.
It has also been caught on camera when kAn and Hyce played, so it can currently randomly strike, but it does seem to be more like the more run time you have on the game.
It doesn't always happen, which is part of the issue when tracking (no pun) down what's causing it. From a bug stamping point of view, rare random bugs like that, are much harder to fix than constant ones that are easy to replicate.
