I've completed two playthroughs, both under 3 hours. But my second game was very different from the first, and both endings I got felt like fail states in very different ways so I imagine someone better at the game can likely go longer and achieve more. Either way it's been very replayable.

You had asked how to increase HP.

1) Don't smoke/drink.

2) Don't get hit in fights, especially not by weapons/bottles.

3) Sleep in a bed, not your car.

4) In school there is a pull up bar, use that.

5) Use food/spa/etc to heal yourself.

The health goes up by like 3/5 every night. At least it stops going down.

To the left of the hotel is a restaurant that appears to heal 200-299 hp. Also there’s the sandwich shop that heals only 99 but it’s free located in the “bar” area on the map.

The restaurant is down stairs to the left of where the hotel is.

The spa is near the right entrance/exit of the majong location. Not the building, just the place on the map.

Essentially, you cannot heal, you can only prevent your character from taking damage. I mean in regards to max health.

My first playthrough lasted twenty minutes or so, but it was quite literally self inflicted.

i tried all this, and it still goes down every day

You dont carry over progress but it does become easier but you cant breeze through
