Long story short, my personal interpretation is that it is just some vaguely described outside gang needed to demonstrate the pressure put on the city's criminal structures. There are several routes in which you can encounter them, and, I guess, if you manage to avoid additional troubles in the hostess girl quest, the countryside sequence is triggered. So it's either you get them or they come for you.
Speaking about Kato, I also thought that having a good relationship with him is required to get help during the shootout. And maybe this kind of fork was initially intended here. But no, currently it's just Kato's willingness to put in work for a common cause (as in the first scenes in which we meet him). The guy is serious about his yakuza career and wants to earn some stripes.
Kato is talking about the girl who owns the stable in which you keep the guy kidnapped in a prior mission.
