This'd be the first time I've heard of this happening so I'd probably need some replication steps to be able to dig into it much further! You might also double check that your Quick Toss key isn't double-bound to any other actions, and try binding it to a different key in a different zone of your keyboard to ensure keyboard ghosting isn't occurring.
I made sure my controllers were unplugged as well, and that didn't seem to have an effect on the bug either way.
The map starts with all of your weapons holstered, and drawing a weapon seemed to usually enable quick toss to work, but 1.) that's not one of the conditions you listed for quick toss to function and 2.) I know that quick toss would suddenly not respond even in the middle of combat.
I'm sorry I wish I could give you more reproduceable steps. I could upload a save and list off my hardware. I do use a custom mechanical keyboard so idk if that has an effect on it, but that would be the first time it has caused any sort of issue like this.
Without reproducible steps or a video of the issue in action, there's not really much I can do I'm afraid. I would maintain that this still sounds like a hardware issue - you might do a ghosting check using some of the keys you'd commonly use during gameplay. For example, hold down two movement keys (W+S), and Shift, and then Z, and see if all of the key inputs are recognized at once:
