In multiplayer games, we often encounter opponents with high ELO and teammates with low ELO.
Everything you leave the game will always exist
Leaver penalty applies at all times, it makes no difference between why you leave. Any match that you quit before a Victory or a Defeat screen could lead to a leaver penalty (even 1v1s, which is a bit silly).
The penalty increases everytime you trigger it, so if you continue to leave games you will get hit with a longer and longer penalty.
There is a grace period, so occasionally leaving one game won't trigger any timeout.
