I had this same question. I have 7 pop, 5 provinces, shouldn't I be able to click another?
My icons are also different sizes for some reason. Is there a cap limit somewhere?
How do I know when a province is available to be annexed?
If your city is unable to annex a province, those icons will be greyed out.
Greyed out when you click territory or greyed out on the hover? I've been doing the click city , click icon thing to no avail and they are colored on the hover (when the econ button in the upper right are checked). I guess I'll try the other option of having the hero in the province.

Side note: I did "purchase" about 2 people via the influence/power thing or whatever it is called.
Cities built underground cannot annex surface-world provinces.
