its a game meant to be fun, if there were 10 minute cutscenes showing his PTSD and mental breakdowns over murdering goons trying to kill him, it would be a very different game

Developers are so aware of that they added the 'ludonarrative disonance' achievement.
Just take it as fiction. If it was a realistic game, Nate would already be dead in the chapter 1, being chased by dozens of armed ships (actually before since the chapter 1 isn't really the beginning of UC4, but you know what I mean). The whole concept of the game, along with Nate's personality and plot armor to tank falls, RPGs and grenades is part of the fiction
