On another note, if your inventory is full and you try to reload, it won't. You need to have at least one spot open that is the size of the magazine.
I've just come back from a long break from the game so I'll try some experiments to see.

Edit 1: Just tried two of the pistols that use 9mm and I had no issues. Now with the .45 Winchester Pistol since there is no clip (if you right click on the gun and hit "Inspect" you'll see there are no spaces to pull anything out of), you'll just need to place your bullets in your open inventory...it won't pull the bullets out of any container.

I always keep more than 1 mag and drag ammo into it and hit "R" always keep ammo in your pockets or armor and you won't have a problem.
what breaks it is where some people store their mags, in other words if you store the mags in the right spots there won't be a problem with reloading any ammo at all.
the armor you can wear also works.
