Random voices, no one is around

I'm hearing random voices while I'm in the middle of no where. I try and find the people but no one is there.

I haven't had that yet but what keeps happening is sometimes I speak with an NPC, or play a voice file from the PDA and the on-screen text from the conversation won't go away. It solved by just saving the game and then exit to main menu afterwards and reload the save.
