When you throw grenades at the NPCs they will stop shooting at you and try to run, then they get knocked down by your grenade because they run in a zig-zag fashion and don't get out of the blast radius. Then when knocked down you throw another grenade to finish them off.
They love to throw grenades on your position and they never miss. If you hear the sound of a grenade landing next to you prepare to lose half of your hitpoints and use the 15 seconds of stun animation to heal yourself.
There is a mod on Nexus that makes the enemies grenades a little bit less accurate.

In Veteran I didnt have much money to repair guns, so I started to throw nades at enemies also- u also have constantly 100 nades at ur disposal.
Nades are meant for thos purpose, to flush enemies out of cover.
Look at how "pushes" are executed in CoD/Warzone- first goes nade into window and then players follow.
True after 1.1.4 the enemy tracking was tuned down and in 1.3 it was back on 1000 but if u crouch and crawl away from ur position u can loose the enemies tracking u.
If u walk or run they can spot u- I mean ure mostly carrying 50-60kg of equipment so u would obviously make noise like pack of elephants while running.
On the other hand when I get Exo I like to sprint at enemis: Thump- thump- thump, " knock, knock, whos there?" " My good friends Mr Texan and Ms Spirit";) and delete them from the zone.
They do use grenades, but not extremely often, you can hear the warning shout a couple seconds before the actual throw, if you look in their general direction you can see the grenade in flight for another 2-3 seconds, you can hear the grenade land nearby (and no, they do not "track" you, they land where they were thrown to, you can easily run to safe distance when you spot them in flight), and even after it landed you still have a good second to take a few steps away that will likely mean you will get severely injured but survive.
To me it feels just right. It forces you out of cover, may injure you if you are too stubborn (or busy with shooting at something) to move when you got all those warnings, but if you actually respond to them as you should, they are not that dangerous. There are far more "unfair" enemy behaviours in the game than grenades from human enemies.
Ive no idea where this "challenge combat" comes from. Maybe I played too little time. Doom gives you fun and challenge.
