Quest line, A BIg Score is confusing and potentially broken?

I know I am supposed to report stuff like this, but is anyone having problem with some quests not getting completed? This one in particular is giving me heart ache right now. I was supposed to see a guy named Akopyan and because the enemies just spam grenades constantly, they tossed one right next to him and he died instantly. I am far beyond this quest at this point, but it's still showing in my Side Quests as uncompleted. Is it just perpetually stuck there because I can no longer proceed with the quest? I also have an issue discerning between completed and failed quests. It is not clear which ones are technically failed. Any help? Thanks!
