So the death count and presumably some other information is stored "somewhere" outside of a save file.
And that "somewhere" might be getting corrupted, and that "somewhere" also holds the names of the save files grouped into each campaign slot. (Bad design - each campaign should instead be its own separate subdirectory, so even if the campaign data gets wiped, the saves in each campaign are still known and the saves for the campaign can be reconstructed by the game by reading the names of the save files in the directory.)
