As an area around the Chernobyl nuclear power plant, of course there are many spots that have very high radiation, many of which are very deadly, so you probably should stay away from these spots until you have more protection. If you encounter too high radiation levels, it won't go away immediately either even when you leave the radiated area, so you'll be "sick" with radiation for a while.
On the other hand, areas like camps where other Stalkers rest, there should be no noticeable radiation. If you encounter radiation there, it's most likely a game bug.
There are artifacts that protect against rads.
You can upgrade gear to add additional protection to rads.
Doesn't really make sense if you're supposed to be seeing through the character's eyes. But neither does film grain, chromatic abberation, lens flare, or many other eye-candy effects that are commonly used in videogames.
