New text walkthrough to 100% game and DLC.

Hello folks, I've just finished a short walkthrough to guide you through the game and DLC while ensuring you get every achievement.
While there's a few guides already I wanted something more involved that clearly explained what and when to do, including pictures, to ensure you don't miss anything so hopefully it might help someone in the future.
