You only need to keep filling rusty tanks. Carry filled tanks in your inventory and the scuba mask will automatically refill when it runs out.

It does work that way (unless you have a bug). You're only supposed to use one scuba gear. Then whenever it runs out, it refills from one of the filled tanks in your inventory. You don't get any empty tanks back from using a filled one. So you have to keep getting new tanks to fill, so you can use them.
I'm not sure if it counts as a clothing item in that sense (even though it goes in a clothing gear slot), but you can always try gifting one to a critter when they ask for a clothing item. :)
