How do I obtain all endings?

With spoiling a bit, there are kind of... 3, but they have 'extra title' variations:
First 2 endings require you to complete the game and are decided by your lewdness... 3rd is game over.

1) 'Pure' ending - self explanatory,. mostly keeping virginities/low lewdness
The more 'virginities', the 'better' you get from this one. This includes mouth for the top tier one. For mouth - any kind of kiss or oral counts, quite sure fingers don't, but not 100% about it.

2) 'Lewd'/'happy' ending - the hardest titles in this one are lewd lvl 400+ and remaining 'technically virgin', if you know what I mean.
In this one you get a special mode where you get 'free roam' to keep playing.

3) Bad ending - game over by reaching order 0.

NG+ is pretty much allowing you to keep most titles(aside of the initially selected one).. Usually the most impactful ones are from main 2 endings to make game a bit easier for you in next attempts.

Do you know how lewd you can go and still get that ending? I'm going for that ending in my current run but I have not been obsessive about it so I'm at lewd level 61. I'm about halfway through the game.

With 61 only halfway in I would say you are on your road for lewd ending, but exact requirements are unknown, so if you are still having her virgin - you can aim for virgin lewd ending... if not - I would go full on lewd and try for 400 lewd... not too sure if it's possible to combine both of those but you could try that as well.

Honestly it doesn't matter too much - the titles you get help considerably less than simply knowing what to do..
Free roam is gotten via the lewd ending, so if you want to bother with that one after the game - it probably does make sense to do lewd 2nd.

I wrote here an optimal start which should pretty much carry you enough to go into any direction you might want. It does suggest the combat abilities for the fighting route, but those can easily be replaced with lewd related stuff too.. you still need mostly same stuff except more focus on mind and maybe some energy.

Is 400 lewd = 400 slut?

slut level is cool, but you dont need 400 to attain happy ending and free roam.
Just beat the captain.
maybe there is a 400 slut achievement.
i have yet to find a path to the virgin/paladin ending, as new game plus seems to make things extremely stacked against that from happening after being a suuuuuper slut queen.
so maybe best to do the virigin route first if you now how.

400 is needed for extra title, not actual ending.. endings are all simple by themselves and are as simple as defeating final boss while being on the lewd or pure side... but have extra titles you can aim for on top of them.

Lewd builds up as your character is exposed to lewd actions and things. Even just looking at the prisoners masturbating raises it. It really is a race to kill the final boss as fast as possible, which is possible if you focus on stat.

No I have my answer already. Lewd level is amount of passives collected.

So I have 399 passives now. Just need to figure out that 1 missing passive for 400.

Correct, yes, can be quite annoying to gather them.
And you can't 'continue', you have to stall the game if you want extra titles from ending(in your case 400 lewdness)... but if you just want to see endings - it's fine enough to just rush the game.
